AFSA Scholarship Applications and Forms
	Select the form below you are interested in save it to your computer so you can complete and submit it to AFSA. For your convenience, the AFSA applications are listed in a PDF and Microsoft Word format. If possible, please submit your AFSA applications via an email attachment to dec@afsa.org using either format. For the documents in PDF format you will only be able to complete them via your computer if you have Adobe Acrobat Standard or Professional. Having only the free Adobe Acrobat reader will just allow you to open and print the form. If you do not have the most recent version of Adobe Acrobat reader, you can download it free by clicking on the icon below. For the Academic and Art Merit Awards: The AFSA Transcript Form needs to be printed and given to your high school guidance office to have a school administrator complete. The AFSA Recommendation Forms needs to be printed and given to or electronically sent to the individual you ask to provide you a recommendation. A one page letter may be sent in with your AFSA Recommendation Form Form. You will need one Transcript Form and two Recommendation Forms for the Academic Merit Application and another set for the Art Merit Application. You also need to submit a two page essay.
